1. Call or text us at 281-484-7619 to get the appointment date and time
2. Once appointment date and time is confirmed, please complete the New Patient Packet. You can download and print from the convenience of your home and the filled out forms can be emailed to info@artxpeds.com or brought in at the time of your appointment.
