 Providing care for your newborn child

Parents need to call and make a “newborn notification” to MAIN MEDICAID (need to call 211) and there HMO/ select plan (ex. CHC, Amerigroup, TCH, etc.) and obtain Reference number and the Name of the person (FULL NAME) they speak with.

WE ASK FOR:

Mothers First Name, Last Name, Date of Birth and Member ID number.

Newborn Notification Reference number and the Name of the person (FULL NAME) they spoke to.

It is very important that parents call 281-484-7619 and notify us with the reference number to avoid any delays in appointment when they are in clinic.

FOR CHIP INSURANCE:

Generally a new born is not covered under mother’s insurance unless a newborn notification is done and reference number is obtained.

FOR PRIVATE/HMO/PPO INSURANCE PLANS:

Please be prepared to provide policy holder’s first name, last name, date of birth, member ID, and group number and also mother’s first name, last name and date of birth.

We have an office policy that if the child is not added to the insurance, the first visit will be $130. Once the child is added and if the insurance makes the payment, we will be to refund the $130.

2nd visit/2 week checkup is $100.00. If the newborn was added and the insurance pays, then this is refunded.
